I'd suggest looking at a CR80 or similar - look for CR, RM, KX or YZ all in 80cc or 85cc versions.

For kit, what fits should be the best bet. If we're not talking immediately, the off road show in a month or two is worth a look, but a long way for you to travel.
If not, get around local shops and try on the kit they have - might be worth trying some on, then looking one ebay.

If you can find some local tracks, give them a shout and ask what they do for kids. A lot will be able to put you in touch with someone that can offer training - though if she can't ride, best bet's probably to take her to some waste ground and show her the basics.

Try looking up fun in the mud online. It used to be done at knockhill but now its up near Kinross/Milnathort. Think you can get a Test-Day Lesson for 2.5 hrs for £60.00.
