Not really.

High-alt detonations especially produce little fallout, as the fireball doesn't touch the ground.

I don't know where you got the number 20 from, but USA dropped way more than that on themselves and they're still alive.

Don't know which is worse, high altitude causing widespread EMP effects destroying the grid et. al. or ground level spewing tons of highly radioactive dust and debris.
My figure comes from an estimation of the actual and collateral damage from an Indo Pakistan nuclear war. Not pretty.
