Ive done the diagram for ya.
You want this made out of 3/4mm steel and you want a 4 ferkin huge bolts. Oh yeah, and some nuts!!
Put The bolts facing up through 2 of the small spacers and concrete these into the floor, leaving enuff bolt on top for the rest to slide onto. Let the concrete go off then...
Slide the rest onto the bolts. It needs to be a sandwich - Big chunk on top, then a spacer at either end then another big chunk. Then fasten the nuts. Then weld them onto the top plate. Then bash them so that they cant be removed.

Try MPS - www.thefastone.co.uk, I bought an Alpha Ground Anchor from them for 36.99 which was easy to fit and does the job. Having just looked in the catalogue though they also do one called the Rotalok In-Turn which costs 40.99.
