just footrests / gear levers aren't they? Or am I missing something?

Sure some look pretty well made and plenty of adjustability etc but they seem expensive. And how much difference will +/- 20mm make for riding on the road anyway?

Why would you ever want aftermarket ones on your bike?

Generally you need them if your pegs are hitting tarmac and you can't hang off any more.
Some will get them because they prefer the riding position.

The rest get them because they like spending money on things and tell themselves everyone else will think they're great because they can do that .
