What goes on an invoice?

Just found that probably the only way to get paid for wasting my time is to invoice the local council.

However, I can't get a proforma invoice from them and I'm not a company so...

What do I need to put on my own invoice?

I guess an itemised bill, name, address, payment conditions (eg pay within 28 days) at the least.
However, what about VAT?
Plus any other (legalish) bits?

Do you have excel, as there is a template in there.

If not, normally you would put your name and address, your customer's name & address, a brief description of the goods or services you are invoicing for and a price. VAT not applicable unless you as an individual or trading company are VAT registered but if you were I would assume you would know how to submit an invoice!

Also give it some sort of unique identifier (eg COUNCIL0001/2009) so that you and they can trace it easily in future correspondence.
