are derestricted 125's trouble or is it better to leave them standard?

Standard is derestricted.

They have been altered by a 'restriction' from their original design.

Most bikes, if anything work better derestricted, though their will probably be more wear on bearings etc, this is the power the bike was designed to have.

Quick bit of free advice; put a post title that reflects the subject you're talking about and you'll get more useful answers.

will putting a different exhaust make much difference instead of doin the full monty deristrict thang?

It's cheaper to just drill out the restrictor plate in the existing exhaust. Assuming that it is restricted there.
It's quite an easy job too!
