How the hell does something like that happen? I imagine it would be standard procedure not to turn on the engine while there's people in the area that could be sucked into it.

Jet engines don't know what a safe distance is, so they paint a red line on the side of the engine and sometimes on the aircraft fuselage to tell humans how close not to get. If you get forward of the line on the engine cowl, or rearward of the one on the fuselage, you better hope the engine doesn't get revved up.
