sounds like the springs in the pressure plate,or the friction plate has become uneven.Most friction plates wear pretty even,so its not that.
Hammering CONNECTED WITH ENGINE REVS is a sign the pressure plate springs have gone,
in the clutch there is a thrust bearing.These dont usually give much trouble,until some stingy mechanic,or greedy garage replace a clutch plate,which is a service item,not a repair.
What they do is fit a new clutch and pressure plate and skimp on the cheap bit,the release bearing.So well before the next clutch plate needs replacing.....the bearing fails....often fekkin up g box as well....
