Money and benefits.

The loyalty to the regime means much easier life for the whole family (you can travel, kids can go to school, get better job in the future... get a car, house....). And disloyalty would result in imprisonment and/or persecution. That is what makes them loyal.

*Or they are just brainwashed.

Nah. It's just something that has been done in various different ways in different countries.

The US had the secret service run along side the car motorcade too. It was shown in that Clint Eastwood film in the line of fire. The PRC does it sometimes as does Japanese PM.

The theory is that it's like tank desant. In old fashioned wars infantry outside tanks could spot and destroy anti tank crews far faster than people inside the tank viewing through narrow view ports.

Guys running along the outside act both as meat shields and observers.
