1) Tick the bit that says you scrapped it yourself, with an approximate correct date.

1) Sign it and send it to the insurance company. If they wrote off the bike it belongs to them. I am amazed they paid anything out without the V5.
2) The DVLA is entirely staffed by spotty, disinterested, (and I suspect illiterate) Welsh teenagers who plug numbers into a keyboard. They will not give a shit if the bike is stolen and they certainly will not check a signature. It is highly unlikely they have even kept a record of it, and if they have, I bet they no-one knows where.
They have been totally unable to spell my name correctly on my driving licence for the last 11 years despite having had five changes of licence, all accompanied by a note explaining their error and having returned it three times outwith that to have it corrected.
