I'm not into extreme body modification, can only speak from the experience I've picked up on the few piercings I've got and hanging around with some people who have had extensive (and in some cases fairly bizarre) work done, and it's like anything else, the motivation is totally individual in each case. For some it's a thrill seeking thing, some just like to feel they're unique, some do it for shock value. Many do it for a mixture of lots of these reasons and others besides, asking "why?" is akin to asking "how long is that piece of string?".
