filling out the provisional license form

hi im currently trying to fill out my DVLA form to get my provisional license and im fairly confused on what i have to tick on ssection one i know i have to tick "a first provisional" bit but i need to know if i have to tick "to add a provisional motocycle entitlement" can someone please just give me a yes or no thanks.

Is there no sheet with the form to explain the different options?

no there isnt the post office women gave me an envelope and the form and i presumed thats all i needed so i took them and went and now cant figure out if im supposed to tick that one box in section 1 other than that everything else is 100% done

awesome thanks Kal so all i need to do is tick "to add a provisional motocycle entitlement" and then i have to wait 15 or more days till my provsional comes then off to my CBT.
