Have you experienced a near miss or accident?
■ Yes ■ No
If the answer is yes, did the incident involve:
■ A rear end shunt?
■ Following too closely?
■ A vehicle driving across your priority?
■ Losing control of your machine?
Did the incident involve a rider/driver (that might include you) in one of the higher than average risk categories:
■ A rider/driver covering more miles than average per year?
■ A male rider/driver?
■ A younger rider/driver?
■ An inexperienced rider/driver?
What have you learnt from the experience?

@swiftb38 - Actually I have found the 'are you a good lover' questionaires extremely useful. As long as you are truthful and don't over analyze them, they are pretty good.
