what the fuck has happened to my poor CBR? Sounds fucking horrendous. Got home, took off my lid and popped out the ear plugs and it now sounds like this.

What mileage. I've read over 10 cases and had 2 cases myself of the bearing on the cam shitting itself around 20k.

Whip the rocker cover off carefully as the bolts are terrible and check it.

just over 12k. Sounds exactly the same as yours

What's it like to replace? easy enough? new engine time?

Easy peasy, Can be done with the engine in frame. Takes 10 mins more than valve clearances

last night I finally managed to get round to whipping the rocker cover off. Bearings on the camshaft are fine. Absolutely no play and spin silky smooth.

What else could it be? Noisy tappets?

I would say the cam chain tensioner is not working correctly.
Check the CCT mounting bolts to make sure they are not loose.
Can you swap it for a known working one, to check it?
