I do most of the time. I ride a lot in London and around cities. I do it on the motorway too if anticipate some close danger.

I put No, in filtering I sometimes cover the rear brake but very rarely the front.

Yeah, cos mostly in London I'm always filtering.

I try not to.
I try and find a gap where I can get off them, predominantly cos it does me wrists in.

But also cos it seems to manifest more stress: I find myself hunching and tensing unnecessarily, so I try and loosen off the controls and relax a bit.

Not normally on motorways, at least, not open ones without incidents.

Servo assisted brake so one finger can apply full force.
Cover front in traffic. (Linked brakes and rarely use the rear.)

Not constantly but, if filtering or when approaching a junction or side road with another vehicle waiting to pull out, then yes.

I feel like covering brake levers is one of these things that novices are encouraged not to do because of all the bad effects panic braking has while cornering or at slow speeds, but that experienced riders do when their spidey senses detect potential trouble ahead.

I cover levers when filtering and when I think I might have to brake hard.

Doing my pathetic excuse for a 'test' here they actually told me off for covering my brake, this is something I've learned from many years riding in the UK and will always do it.

Clutch not so much, I don't really use it up or down the box

Probably otherwise similar to most others on here - cover the levers and rear brake when filtering/spidey sense tingles, otherwise fingers loosely wrapped around the grips .

For the most part no, again spider sense proviso.

If something startles me when covering the brake I'm going to squeeze it. Birdstrike and steathwasp included. Moving my hand ensures I come off the throttle and then progressively apply the brake, and my old cruiser needs a hefty four finger squeeze to achieve maximum braking, but will still lock wheels if I snatch. If I had mad skillz and nothing ever startled me it might allow a faster reaction time. I am not that awesome.

Training myself not to chop the throttle as a panic reaction mid corner was hard enough.

Whilst I'm aware I'm not wired like most others, it's pretty rare to find a human that can be primed for action for more than a few seconds without the chance of a fuckup occurs.

Similarly fingers do not go on triggers unless you have a target and are ready to shoot.

Yes I do and it resulted in quite a few of my accidents when I first started riding, however It doesn't bother me.
