The little monster (cg125) is currently in several bits... got it a new spark plug and cap, and its giveing a nice phat spark, for sayng its been in a shed for two decades and was last started in 1990 or so i'm pretty impressed...

Anyway, for about 10 years its not had a sparkplug (or cap) or exhaust, i get the feeling at least some dirt, could be rust, could be hay (dont ask) could be in there, the cyclinder move 100% freely no problem, i've filled it up with redex and left that in there a few days, i'm hopeing that would have gotten rid of any significant stuff.. there might be none in there at all but i couldn't say....

should i worry or will it probably be fine? what could i expect to happen at the worst? knackered piston?

If you've got the oil out and are doing other things to the engine, pop the head off, that way you can see both piston and barrel also allowing you to check if they valves are ok as the exhausts can get blocked slightly open with carbon.

If you need to strip the engine apart again because it requires a thorough cleaning then use a product called 'Gunk' because it cleans and degreases the engine parts. Just dry it off with a lint free cloth to remove the last of the Gunk prior to assembling the engine.

Drain the oil. Turn it upside down, take the plug out and give it a good long blast opr two of carb cleaner spray up through the sparkplug hole. After it has drained and dried out, turn it back onto its wheels, put a few drops of oil down into the bore, refill with engine oil. Kick it over a good few times to get the engine oil circulating and replacing what you blasted out with the spray. Chances are that any junk would be blown straight out of the exhaust valve anyway.

NB: I have not tried this before, just working on the basis that it is a cheap CG125 and that would be a simple way to get any shit out of it.
