AFAIK it's just your license and pass certificate you need to send off. Your problem is that if DVLA spack it up, then you've got no record of passing. Really, you're screwed, they're notorious for not accepting copies or budging one iota.
You could pop down to the DVSA test centre with your docs and a cheery smile, and see if they'll push the button now. Probably not.

Why it's important that DVSA Derek shreds a piece of plastic (he doesn't "send it off") before he pushes the button escapes me. The plastic card is just ID now, it's not definitive.
What's definitive is https://www.gov.uk/view-driving-licence - have you had a look on there? Perhaps by some miracle they might have actually done it. ____________________ Biking is 1/20th as dangerous as horse riding.
