Strange indicator problem

Since I changed the indicators on the NSR to mini ones, I changed the relay unit aswell to account for the higher wattage bulbs. However, after a few months they started blinking at strange rates and then failed to come on at all. I changed the relay unit again and all was well up until tonight, exactly the same problem. Any ideas how I could resolve this? Anyone know what the numbers mean on the side of the unit? It says something like 2 x 12v + 5w. I can only assume they mean 2 lights at 12v with 5w tolerance. (it's a car relay from Halfords Rolling Eyes )

Or, 2x12w indicator bulbs 1x5w dash light?

I'm not too clued up on indicator relays but I would presume all the indicator bulbs need to be 12w and the dash light needs to be 5w to match the relay.
