If it's really rusted I'd replace it. Though that can get expensive if lots are fucked.

It shouldn't be critical if the rust is on the lip of the piston and never goes past the seals. It is most likely to need the seal grooves cleaning, for which a dremel (with a reasonably soft brush - brass) is perfect.
If you use a dremel on the piston avoid using a stone and try not to scuff up the clean surface.

Is it rust or dirt?

A friend needed one new piston for one of his four piston calipers on his GSXR750 a few years ago.At the time they were really expensive as OEM parts,so I asked a machine shop to make one out of a decent grade of stainless steel.

Cost was about £5 at the time Thumbs Up

as has been said, might consider a re-build / refurbishment kit of new seals and piston depending how bad it is

otherwise brass brush is softer than the piston so should be ok

I use a stainless-steel scourer for most surface rust jobs, because it doesn't break-up like ordinary wire wool.

On a caliper piston though, if it has rusted below the line where it contacts the seal, I would bin it and get a new one - it's not worth the risk.

This.

Are you sure its not just a build up of road crap, try giving it a good scrub with hot soapy water and a toothbrush first.

cool , was just concerned if I can use a wire brush on it.

is it not like car piston which any scratch or uneven surface can mess it up ?
worst case scenario, big scratches on the outside of the piston, does it let fluid out ?

It works just like a car brake piston and requires just the same kind of cleanliness. And yes, big scratches in sides will leak fluid.
