They are hollow tubes of metal with specially designed width's, lengths and with baffles inside. The amount and type of baffleing combined with the can dimensions effect how freely the gasses can escape.
Generally a 'Sports' exaust has far less baffleing and gives improved performace.
An after market end can is normally the cheapest, quickest way of getting more power out of your bike, allthough the extra power gained varies from bike to bike.

Also, with 2 stroke exhausts, the way the expansion chamber is designed can determine how the power is produced, i.e. more torque or more top end power. Baffles in a 2 stroke race can also have to be specially designed to keep in backpressure, yet at the same time flow as much gas out as possible.

For new bikes, I would say an aftermarket can is a way of making it reasonably louder.
Not the cheapest, but may be quicker than cutting out the baffels from your original can.

If you look at the figures, rarely do aftermarket cans add that much bhp, especially to newish bikes.
Those that do add decent power are usually full systems and require fueling changes to make the most of them.

It depends what you mean by 'performance'; look at new prices for an OE system and an aftermarket one and the original may well cost more.

So, in my opinion, unless you're willing to go for a full system with jetting changes, the main difference is the 'performance' cans are louder.

There are exceptions to the rule, for instance some bikes have intentionally quite restrictive exhausts as standard.
