I use whatever the cheapest earbuds i can get my hands on are.
Over the ear ones hurt like fuck
Regular earphones fall out or get ripped out .

Sennheiser CX 300-II Precision Headphones
Was a thread a while ago and they seemed to be the majority vote. Had mine for about a year before they finally went whilst cycling in a storm. Don't hurt your ears at all because they're small and rubbery, great sound quality and cheap enough to quickly replace if they break. my Sennehseisers broke after a couple of years, bought these for £10, bass is very good and cuts through wind noise well. I normally wear them with the wire going up and around the top of my ear flap, then going into my ear upside down, go in and stay in fine but sometimes pop out when removing the helmet.

I cant stand shit sounding headphones. The ones that come with the iPhone hurt my ears if I wear them for longer than 20mins and as you say they fall out.
I used to uses these

Lasted about two years of wear everyday.
Pros: easy on the ear, sounds good but ear canal could still breathe. All working iPhone remote.
Cons: Expensive, sometimes fall out in helmet.
Then I got,

Last about 6months of daily use.
Pros: Cheaper, Lots of bass, never fall out, great isolation if you like that, great for motorways, easy RMA process . All working iPhone remote.

Cheap ear buds, with silicone gloop to mould them to my ear. ____________________ Biking is 1/20th as dangerous as horse riding.

Keeps the wind moise out and I can have the music at any volume!
When my girl friend moans and nags at me in bed I just put these babies in and look at and whisper: "Problem!"
I didnt want to spend that much my budget was originally £70
